The Western and Southern Life Insurance Company’s (Western & Southern Life’s) Critical Illness Insurance* helps fill the financial gaps not covered by traditional health, disability, life and accident insurance.** If a life-threatening illness occurs, our Critical Illness Insurance pays the insured a benefit to help protect his or her quality of life. Critical Illness Insurance is an individual health policy that pays a lump-sum benefit upon diagnosis of one of the covered illnesses as defined in the policy. A partial benefit may also be paid for a specific illness or specified surgical treatment as defined in the policy. The issue ages for Critical Illness Insurance policies are 18-69. (The maximum issue age in California is 64.) Our Critical Illness Insurance pays you the benefit amount you select – from $10,000 up to $100,000 – upon diagnosis of a qualified critical illness.

The Critical Illness base product has a 50 percent benefit reduction at the policy anniversary on or following the insured’s 65th birthday or five years after issue for issue ages 60 and above.

In some states, Critical Illness Plus is available. The Critical Illness Plus product does not have a reduction of benefit amount.

The policy benefit amount is paid in a lump sum. Critical Illness Insurance gives you choices because your money can be used for anything you please. You may want to use the money to:

Pay off debts.
Supplement lost income.
Take advantage of experimental treatments.
Arrange for child care while recovering.
Take a trip.
Maintain your quality of life.

Covered Illnesses
Western & Southern Life’s Critical Illness Insurance pays upon first diagnosis 100 percent of your maximum benefit if you***:

Have a heart attack (myocardial infarction), as defined in the policy.
Are diagnosed with life-threatening cancer (malignant neoplasm), as defined in the policy.
Have a stroke, as defined in the policy.
Go on a recipient list for a major organ transplant.
Are diagnosed with end-stage renal failure.
Western & Southern Life’s Critical Illness Insurance pays 25 percent of your maximum benefit if you***:

Have a first coronary angioplasty (surgical treatment).
Are diagnosed with a malignant tumor (first carcinoma in situ), as defined in the policy.
Have first coronary artery bypass surgery.

Partial Benefits
After payment of a 25% partial benefit, the face amount reduces to 75% and the premium reduces to that which is appropriate for the new face amount. Payment of a second 25% partial benefit reduces the face amount by 25% of 75% and the premium reduces to that which is appropriate for the new face amount. Payment of a third 25% partial benefit reduces the face amount by 25% of the face amount at that time and the premium reduces to that which is appropriate for the new face amount.

Return of Premium
What if the insured is not diagnosed with cancer and does not suffer a heart attack, stroke or any other covered illness? Upon the insured’s death if the policy is in force, we will return to the owner or to the owner’s beneficiary if the owner is deceased or to the owner’s estate if there is no beneficiary, 100% of all premiums (excluding Accidental Death Benefit) paid minus any benefits paid. The premiums to be returned will be calculated: 1) without interest and 2) after all pending claims have been settled. If the sum of the benefits paid is equal to or greater than the sum of the premiums paid, there will be no return of premium.****

Premiums & Renewability
Premiums are based on age at issue and are intended to be level throughout the insured’s life. However, Western & Southern Life has the option to file for rate increases on a state-by-state basis if experience dictates such action is necessary. Critical Illness Insurance is guaranteed renewable for life without evidence of insurability.

Available Riders
Western & Southern Life offers one rider for Critical Illness Insurance. An Accidental Death Benefit (ADB) Rider, available for issue ages 18-60, pays a death benefit to the policy’s beneficiary if the insured’s death is a direct result of an accident, as defined in the policy. The rider is not available in all states. This rider is available at an additional charge.

Companion Policy
Rather than offering spouse riders, we offer companion policies. The companion policy applies to the spouse only and provides that if both Critical Illness Insurance applications are received at the same time, we will reduce the policy fee from the normal $50 per policy to $25 on each of the policies.

Underwriting Process
The underwriting process for Critical Illness Insurance involves an acceptance or rejection based on the answers to the questions on the policy application. An applicant is either accepted and issued a policy or rejected and denied coverage; no additional medical records or tests are required to determine eligibility. More than one policy on an individual will be permitted as long as the total Critical Illness Insurance coverage does not exceed the maximum face amount of $100,000. Individuals with Cancer plans or Life plans may purchase the Critical Illness Insurance plan.
